INFORMATION ASSESSMENT FOR THE PREPARATION OF A STABILITY GUIDE FOR INJECTABLE MEDICINES AFTER RECONSTITUTION AND DILUTION IN A MATERNAL-CHILDHOOD HOSPITAL

Abstract


Weaknesses in the administration process mean that the last patient safety barrier, in which errors can be intercepted, has been broken. Therefore, safe  and effective strategies must be inserted in the institutions to guarantee the  quality of health care by reducing medication errors. The study investigates  adequate and standardized information on the stability of drugs for post- dilution parenteral administration to prepare a guide with relevant guidance  in this regard. We carried out a search and comparison between the agreed list of drugs used at Seridó’s Hospital and available technical-scientific information from highly reliable sources. (Micromedex®, UpToDate®). A full  of 89 drugs were analyzed, and technical information was provided on their  parenteral use. From them, 64 (71.91%) can be involved in drug  incompatibility if administered in a Y-site device concomitantly. Few studies  address this issue, which also gives relevance to the study. Ensuring the  provision of technical-scientific and independent information about the drug  is the pharmacist’s responsibility, contributing to avoid errors, consequently  improving patient safety.
